    Neurocognitive Monitoring and Care During Pediatric Cardiopulmonary Bypass—Current and Future Directions

    Neurologic injury in patients with congenital heart disease remains an important source of morbidity and mortality. Advances in surgical repair and perioperative management have resulted in longer life expectancies for these patients. Current practice and research must focus on identifying treatable risk factors for neurocognitive dysfunction, advancing methods for perioperative neuromonitoring, and refining treatment and care of the congenital heart patient with potential neurologic injury. Techniques for neuromonitoring and future directions will be discussed

    Mercury Methylation by Cobalt Corrinoids: Relativistic Effects Dictate the Reaction Mechanism

    The methylation of HgII(SCH3)2 by corrinoid‐based methyl donors proceeds in a concerted manner through a single transition state by transfer of a methyl radical, in contrast to previously proposed reaction mechanisms. This reaction mechanism is a consequence of relativistic effects that lower the energies of the mercury 6p1/2 and 6p3/2 orbitals, making them energetically accessible for chemical bonding. In the absence of spin–orbit coupling, the predicted reaction mechanism is qualitatively different. This is the first example of relativity being decisive for the nature of an observed enzymatic reaction mechanism.Of relative importance: The methylation of HgII(SCH3)2 by corrinoid‐based methyl donors proceeds in a concerted manner through a single transition state by transfer of a methyl radical.     Coded apertures for x-ray scatter imaging

    We examine coding strategies for coded aperture scatter imagers. Scatter imaging enables tomography of compact regions from snapshot measurements. We present coded aperture designs for pencil and fan beam geometries, and compare their singular value spectra with that of the Radon transform and selected volume tomography.We show that under dose constraints scatter imaging improves conditioning over alternative techniques, and that specially designed coded apertures enable snapshot 1D and 2

    Time Required for Lameness Detection on an Embedded Microcomputer Based Force Plate in a Lab Based Setting

    The objective of this study was to determine the minimum time required for the embedded microcomputer based force plate (force plate) to detect lameness. The force plate can be placed under an electronic sow feeder or a gestation stall to routinely assess lameness. Previous work with the force plate has required sows to remain standing in a gestation stall for longer than a typical feeding time allotment of 9 minutes to measure the force applied to each foot. Comparing sows’ weight distribution across time showed that an adjustment period is required before force distribution measurements are consistent, as the sows adapts to the force plate. When using a 30 second adjustment period, sows only needed to stand on the force plate for 210 seconds in order to have a consistent reading across time. This could create a labor reduction during research trials, and allow more data to be used from a commercial setting, than when using the previous times

    The EX-FRAIL CKD Trial: a study protocol for a pilot randomised controlled trial of a home-based EXercise programme for pre-FRAIL and FRAIL, older adults with Chronic Kidney Disease

    Introduction Frailty is highly prevalent in adults with chronic kidney disease (CKD) and is associated with adverse health outcomes including falls, poorer health-related quality of life (HRQOL), hospitalisation and mortality. Low physical activity and muscle wasting are important contributors to physical frailty in adults with CKD. Exercise training may improve physical function and frailty status leading to associated improvements in health outcomes, including HRQOL. The EX-FRAIL CKD trial aims to inform the design of a definitive randomised controlled trial (RCT) that investigates the effectiveness of a progressive, multi-component home-based exercise programme in pre-frail and frail older adults with CKD. Methods and Analysis The EX-FRAIL CKD trial is a two-arm parallel group pilot RCT. Participants categorised as pre-frail or frail, following Frailty Phenotype assessment, will be randomised to receive exercise or usual care. Participants randomised to the intervention arm will receive a tailored 12-week exercise programme, which includes weekly telephone calls to advise on exercise progression. Primary feasibility outcome measures include rate of recruitment, intervention adherence, outcome measure completion and participant attrition. Semi-structured interviews with a purposively selected group of participants will inform the feasibility of the randomisation procedures, outcome measures and intervention. Secondary outcome measures include physical function (walking speed and Short Physical Performance Battery), frailty status (Frailty Phenotype), fall concern (Falls Efficacy Scale-International tool), activities of daily living (Barthel Index), symptom-burden (Palliative Care Outcome Scale-Symptoms RENAL) and HRQOL (Short Form-12v2). Ethics and Dissemination Ethical approval was granted by a National Health Service (NHS) Regional Ethics Committee and the NHS Health Research Authority. The study team aim to publish findings in a peer-reviewed journal and present the results at relevant national and international conferences. A summary of findings will be provided to participants, a local kidney patient charity and the funding body
